New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Please note you will need to come into the surgery with 2 forms of identification before your registration can be completed.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.

Changes to Personal Details
If you change name, address, email or telephone number, please let our receptionists know by notifying us in writing. This helps us to contact you when necessary and maintain your integrity. If you move outside the practice area you may need to find a doctor in your new area. There is a form which will need to be filled out that can be collected from Reception in order to change name or address. We cannot change your name or address without this form being filled out. We also ask that you bring in proof of the change. We cannot complete the change without proof. We do not need any proof for your email or telephone number.
